										<content:encoded><![CDATA[<p>Our toenails tell us a lot about our overall health and well-being.  Nails contain a protein called <em>keratin</em>, just like skin and hair. This is what makes nails strong, hard plates or shields.  The <em>matrix</em> in the nail bed creates new skin cells which push out the old ones creating a nail.  If the nail matrix is damaged or injured, this can affect the way the nail grows.</p>
<p>To properly deal with toenail problems, it’s important to know what has caused them to become dry, brittle, discoloured, crumbly, smelly or painful in the first place.  Let’s explore the different causes and what can be done to help!</p>
<h3><strong>What Can Cause Dry, Brittle or Discoloured Toenails?</strong></h3>
<h4><strong>Nutrient deficiencies</strong><strong> can affect the texture and appearance of toenails.  </strong></h4>
<ul>
<li>Low calcium and vitamin C may result in thin, weak, and brittle nails.</li>
<li>A vitamin B12 or iron deficiency may cause the nails to become white, thin, and brittle.</li>
</ul>
<h4><strong>Certain medical conditions influence the health of our toenails.  </strong></h4>
<ul>
<li>Diabetes may make a person more susceptible to fungal infections and can also affect the blood flow to the feet and toes, reducing necessary nutrients and oxygen.</li>
<li>Rheumatoid arthritis causes joint inflammation and toe deformities which often results in excess pressure on toe and nails.</li>
<li>Psoriasis can cause pitting in the nail plate and may result in a rough surface texture.</li>
<li>HHypothyroidism, or an underactive thyroid can make nails brittle.</li>
<li>Lichen planus is an inflammatory skin condition that can cause thinning, ridges and splitting of the nail plate.</li>
<li>Chemotherapy or radiation may also change the shape and texture of nails.</li>
</ul>
<h3><strong>What Causes Ingrown and Fungal Toenails?</strong></h3>
<p>An <strong>ingrown toenail</strong> occurs when one or both sides of the nail digs into the surrounding skin.  It may or may not pierce through the skin but can result in pain and discomfort and may cause redness and swelling. If left untreated, ingrown toenails can result in significant pain and infection and could force you to change the way you walk or move to protect the affected toe. <strong>Fungal toenails</strong> are common and can be uncomfortable, thickened, misshaped, discoloured, crumbly and smelly.  Contagious in nature, they can spread from one person to another or from one toenail to another.</p>
<ul>
<li>Ill-Fitting Footwear that is too tight or small can cause microtrauma to the toes and nails, increasing the risk for ingrown and fungal toenails.</li>
<li>Cutting toenails too short or ripping them off may leave a spike of nail in the corner.</li>
<li>Toenail injury from trauma can inadvertently cause the nail to dig into the surrounding skin or it can damage the nail matrix increasing the risk for a fungal infection.</li>
<li>Toe deformities, such as hammer toes and bunions change the position of toes, leading to increased pressure and injury to the nail.</li>
<li>Genetics may influence the shape or curvature of the nail. This curvature can be so severe that the sides of the nail come together underneath and pinch the tissue of the nail bed, also known as a pincer nail.</li>
</ul>
<h3><strong>How Can Toenail Problems be Addressed and Treated? </strong></h3>
<p>As mentioned above, our toenails are often the gateway to our overall health and well-being.  It’s crucial to be aware of any systemic issues that need medical attention.  If your toenail problems are related to microtrauma, ill-fitting footwear, toe deformities or an exaggerated curvature of the plate itself, BioPed Footcare can help!</p>

										<content:encoded><![CDATA[<p>Fungal toenails are common and can be uncomfortable, thickened, misshaped, discoloured, crumbly and smelly. Also known as <em>onychomycosis </em>(on-ih-koh-my-KOH-sis), toenail fungus can be a source of embarrassment and insecurity and can even lead to skin breakdown due to excess pressure on the nail bed. Contagious in nature, they can spread from one person to another or from one toenail to another.</p>
<p>Toenail fungus is evident in the toenail itself, but the infection resides in the nail bed underneath the nail plate. The nail matrix or cells from which the nail grows is the part of the toenail that becomes infected with fungus. If toenails are injured or if a person’s immune system is not working as well as it should, there is a greater risk for a fungal infection developing.</p>
<h3><strong>Fungal Toenails &amp; Important Things to Remember</strong></h3>
<p>Fungal toenails can be hard to eradicate. The challenge with treatment is penetration of the medication, as it must get through the nail plate down to the nail bed where the fungus lives. The thinner the nail plate, the better a topical treatment will work! Before starting any anti-fungal treatment, it is recommended to know if your toenails are in fact caused by a fungus. Not all thick and discoloured toenails are fungal and not all fungal toenails are thick and discoloured!  BioPed Medical Footcare Clinicians can work with your Primary Care Provider to help determine the answer and the best next steps.</p>
<p>Fungal toenails can be treated with topical medications applied directly to the nail and/or oral medications, which are prescribed by a Medical Doctor, Nurse Practitioner or a Chiropodist. These oral medications are quite powerful and can have undesirable side effects and interactions with other medications.</p>
<h3><strong><u>Treatment Options for Onychomycosis</u></strong></h3>
<p>BioPed Footcare clinics offer an array of treatments for toenail fungus. We offer topical treatments such as paint on anti-fungal medication and <em>Photodynamic light therapy </em>(PDT).</p>
<p>At-home treatments may be prescribed or purchased over the counter. <em>Jublia</em> is a well-known prescription treatment. Over-the-counter options include <em>Footlogix Nail Tincture </em>and<em> Formula 3</em>. Formula 3 contains the active medicinal ingredient, <em>tolnaftate</em>. Tolnaftate has been used in products for over 50 years to treat fungal infections, such as ring worm and athlete’s foot. Formula 3 uses jojoba oil as the base or the vehicle that delivers the tolnaftate, creating an <em>in-solution</em>, NOT a suspension. This prevents separation of the ingredients and allows immediate absorption of the oil with the tolnaftate right alongside! Both options are available at BioPed and can be used to treat and/or promote healthy nail growth.</p>
<p>In-clinic treatments such as PDT are highly effective treatment options for fungal toenail infections. At BioPed, we use the <strong><em>ClearToe Therapy Light</em></strong> specifically, which is a class 2 photodisinfection device approved by Health Canada for the clearance of fungal nail infection. ToeFx® has a proprietary non-toxic, ClearToe Serum which when applied to toenails, binds to fungus and becomes concentrated with the light. Toenails are exposed to the light therapy for 15 minutes each session. Sessions are biweekly for 4-5 months. Studies out of McMaster University, revealed this ClearToe Light Therapy system resulted in complete or substantial clearance of toenail fungus in 76% of patients after 8-10 sessions.</p>

A portion of the nail is adhered to the nail bed and a portion at the end, the free edge, is the part we trim. If the nail matrix is damaged or injured, this can affect the way the nail grows. The nail may become thick which will cause discolouration, or it may become weak causing it to break easily or appear dry and rough.</p>
<p>To know how to properly treat a dry toenail, it is important to know what caused it to become dry, brittle or discoloured in the first place. Let’s explore the different causes and what can be done to help.</p>
<h3>Footwear</h3>
<p>Ill-fitting footwear can cause repetitive trauma to the nail which leads to damage over time. BioPed’s lower limb clinicians will make sure you are wearing the proper size and style for your feet and activity. A wide and deep toe box allows toes to move during gait and reduce pressure and injury to the nail plates. Supportive footwear will offer stability to improve movement of the feet which also lessens injury to the toes and nails.</p>
<h3>Toenail Fungus</h3>
<p>Fungus is a hot topic these days as it is contagious and common and can be unsightly! A fungal infection in the nail appears in the nail plate but resides in the nail bed’s matrix. Fungal nails can vary in appearance but are often thickened, crumbly, yellow, or brown and may have an odour. Treatment should include thinning of the nail plate to increase comfort in footwear and allow more successful results while using over the counter or prescription medications applied to the nail. There are also oral medications and light therapy. It is important to be assessed by a medical footcare clinician to learn the best treatment option for you, as not all treatments are appropriate for everyone.</p>
<h3>Diet</h3>
<p>Nutrient deficiencies can affect the texture and appearance of toenails. Low calcium and vitamin C may result in thin, weak, and brittle nails.  A vitamin B12 or iron deficiency may cause the nails to become white, thin, and brittle. Your primary care professional can check your blood to rule out or confirm a nutrient deficiency.</p>
<p>Certain conditions influence the health of our toenails. Those that negatively affect our body’s immune system or joints can lead to issues. Diabetes may make a person more susceptible to fungal infections and can also affect the blood flow to the feet and toes reducing necessary nutrients and oxygen.  Rheumatoid arthritis causes joint inflammation and toe deformities which often results in excess pressure on toe and nails. However, if you are struggling with more skin and nail-related concerns, such as ingrown toenails, calluses, corns, or warts, seek the expertise of a Registered Chiropodist or an Advanced Footcare Nurse.</p>
<h3>But when should you opt for a chiropodist vs a footcare nurse?</h3>
<p>In simple terms, choose a BioPed Registered Chiropodist when you require orthotics, a prescription (for medicine or orthotics, shoes or bracing), have an infection, have a foot wound, may require surgery or numbing (by injection), or are neuropathic (have little to no sensation in your feet). Chiropodists can prescribe oral and topical medications and administer injections into the feet. They can also offer surgeries on skin and toenails.</p>
<p>An Advanced Footcare Nurse provides trimming for calluses, corns, thick or ingrown toenails, and treatment for foot warts. They perform most of the same skin and toenail treatments as a Chiropodist, except for surgeries, injections and prescriptions.</p>
<h3>If you are suffering from any of the following conditions, then medical footcare at BioPed is the right service for you!</h3>
<h4><strong>Fungal toenails</strong></h4>
<p><strong>Fungal nails can be thick, misshapen, discoloured, crumbly and smelly. They can occur on the big or small toes and are treated with regular debridement, topical or oral antifungal medications.</strong></p>
<p>For topical solutions to be effective, the nail must be thin enough for the treatment to penetrate to the nail bed where the fungus resides. A Chiropodist or Footcare Nurse can significantly reduce the thickness of the nail plate. If necessary, a Chiropodist can remove the nail plate altogether.</p>
<h4><strong>Ingrown toenails</strong></h4>
<p><strong>A painful toenail border (or edge) due to the nail growing into the soft tissue is considered an “ingrown toenail.”</strong></p>
<p>You may experience pain when the nails are significantly curved or when a spike of a nail punctures the skin at the side of the nail. Ingrown toenails usually respond well to correct nail cutting techniques, properly fitted footwear and patient education.</p>
<p>BioPed Chiropodists and Footcare Nurses can opt to apply a toe<em>nail brace,</em> which helps alleviate pressure at the sides and retrains the nail edges to grow straighter. Registered Chiropodists can also perform toenail <em>avulsions</em> to remove the offending nail edge and prevent a recurrence.</p>
<h4><strong>Corns &amp; calluses</strong></h4>
<p><strong>Corns and calluses appear as thickened skin on the bottom or tops of the feet and in between the toes.</strong></p>
<p>Corns and calluses can be painful because of the high pressure or friction caused when skin builds up in local area. To offset the pain in the feet, a person might alter how they walk, creating other issues, such as knee pain.</p>
<p>BioPed Chiropodists and Footcare Nurses will trim corns and calluses and recommend a topical moisture regime that will help with hard and dry skin. Chiropodists can also prescribe custom foot orthotics, which can correct gait and offload high-pressure areas, reducing the corn / callus pain and need to return for frequent trimming. BioPed clinicians can also recommend properly fitted footwear appropriate for specific activities and your foot type, to increase comfort and function.</p>
<h4><strong>Plantar warts</strong></h4>
<p><strong>Plantar warts are found most commonly on the bottom of our feet and are contagious.<em> </em></strong></p>
<p>Warts have a cauliflower-like texture, black dots centrally, and are usually painful when lateral pressure is applied. They are caused by the<em> human papillomavirus</em> (HPV) and can be challenging to cure.</p>
<p>Registered Chiropodists and Advanced Footcare Nurses will debride wart tissue to allow chosen treatments to work better. This could include topical solutions such as salicylic acid and Cantharidin. In addition to conservative treatments, Chiropodists can also perform <em>wart needling</em> and <em>wart excision.</em></p>
<h4><strong>Toe deformities</strong></h4>
<p>Hammertoes, claw toes and mallet toes are all forms of toe “deformities”. They refer to toes that bend at one or both toe joints, creating a toe that is not straight. They can cause calluses, corns, pain, and discomfort, making finding the right footwear a seemingly impossible task.</p>
<p>To address toe deformities, a silicone sleeve may be recommended, or Chiropodists can make a custom toe prop, which can help straighten the toes and prevent rubbing, when weight-bearing. BioPed clinicians will also address any calluses or corns that have arisen due to the toe deformity and makes recommendations for footwear.</p>
<h4><strong>Diabetes &amp; foot health</strong></h4>
<p>People who have diabetes are more susceptible to infection, cold feet and other complications, such as <em>peripheral neuropathy</em>. To ensure proper foot health is maintained, BioPed Chiropodists and Footcare Nurses will perform routine nail and skin care. Diabetic Risk Assessments are 3-minute evaluations performed in clinic to determine a person’s risk for foot complications. Following this evaluation, the clinician will provide education on how to inspect feet at home in between appointments to ensure they stay healthy.</p>
<h4><strong>Biomechanical treatment</strong></h4>
<p>Registered Chiropodists are trained in assessing gait (walking pattern/style) and the quality of movement of the foot and lower limb joints and muscles. Depending on the lower body mechanics, a Chiropodist may prescribe custom foot orthotics to correct alignment and/or provide protection to the foot or improve the way someone walks or runs.</p>
