										<content:encoded><![CDATA[<p>Our feet are a complex structure of muscles, tendons, ligaments, bones, and joints that must all work together for proper function.  If feet are abnormally positioned or alignment is off, the rest of the body could suffer.  Imbalance can also lead to our muscles compensating to attempt to regain balance and function.  Custom made orthotics or prefabricated over-the-counter insoles can effectively treat issues in the feet and therefore treat pain and discomfort further up the chain.</p>
<p>When standing, the body is a <em>closed kinetic chain</em>, meaning what happens in the feet can affect the ankles, knees, hips and back.  If the feet are abnormally positioned or their alignment is off, the rest of the body could suffer.  Let’s investigate lower body issues that could benefit from a pair of orthotics!</p>
<h3>Ankle Pain</h3>
<p><em>Overpronation</em> is usually associated with a flat foot type and <em>over supination </em>with a high arched foot.  Both foot types can cause increased strain or compression on the structures surrounding the ankle joint.  A<em>nkle joint instability</em> is a common issue encountered at BioPed Footcare.  It can be a result of weakness around the ankle or a history of frequent ankle sprains.</p>

<p>The position of the heel plays an important role in the alignment of the rest of the lower body, including the knees.  Foot pronation will put more strain on the inside or medial structures of the knee including tendons and ligaments.  Supination reduces the amount of motion in the foot, which reduces the amount of shock that can be absorbed with impact.</p>

<p><em>Bursitis</em> and <em>arthritis</em> are common issues that affect the hip joint. <em>Bursitis </em>develops when a fluid-filled sack around the joint called a bursa becomes inflamed.  Bursae cushion the bones, tendons and muscles around your joints and can become inflamed when overused or an injury occurs.  <em>Osteoarthritis</em> is a degeneration of the cushioning cartilage in the joint.</p>
<h3>Lower Back Pain</h3>
<p>Pain in the lower back is a common struggle for many people.  Back pain can cause us to adjust our gait in order to move about more comfortably, causing a domino effect in our body’s closed kinetic chain.   On the flip side, overpronation and hypermobility in the feet increases internal rotation of the legs and excessive supination decreases range of motion in the feet, leading to more impact travelling up the body to the lower back.</p>

<p>You might be asking “So why is shoe width important?” Every foot is unique, just like every person. Some of us have narrow feet, while others have wider feet. Just like wearing pants that are two sizes too small, or two sizes too big; shoes that are too narrow or too wide can spell trouble for your feet. Neither scenario is ideal for the comfort or health of your feet.</p>
<p>Your feet need the right support to carry you through life’s adventures. Not only can shoe width make you feel uncomfortable, but it can also affect the health of your feet. If the shoe is too narrow, you will feel like your toes are getting squeezed together. Consistently wearing too narrow of shoes can cause ingrown toenails, painful blisters, calluses or corns, and can even lead to bunion formation or hammertoes. On the flip side, wearing shoes that are too wide can cause your feet to slide around in the shoe, leading to blisters, instability and potential injuries like sprains.</p>
<h3>Picking the Right Shoe</h3>
<p>When looking for a shoe, opt for one that has a slightly larger toe box, one that allows you to wiggle your toes freely, but not so much so that your foot is sliding around. Delicately squeeze your foot when in the shoe to see if there is any gapping in the shoe material. If there is, the shoe is too loose, opt for a narrower width. If your foot is bulging over the sides of the shoe, the shoe is too narrow and opt for a wider width. Some brands of shoes only come in one width, so if you tend to have a narrower foot or wider foot, try to avoid those specific brands.</p>

Shoes with such little or no shock absorption allow the pressure from hard surfaces like concrete and tile to transmit into your foot. While this might feel fine at first, repetitive heavy pressure on your heels and the balls of your feet can aggravate foot conditions and lead to pain down the road. Reducing excess pressure to your foot can be crucial for those suffering with arthritis and stiff joints, and protecting the skin from building up calluses is a top priority for those with diabetes.</p>
<h3><strong>Narrow and tight fitting</strong></h3>
<p>These shoes prioritize a slim fit right up to the toes for a streamlined aesthetic. While those with narrow and low volume feet might be able to fit into this shape, most people require a medium to wide width shoe. Many people require even extra width and or depth to best fit their foot shape, and most athleisure shoes do not offer sizes to accommodate this kind of fit. On top of this, the toe box is very tapered and pointy, leading to a tight fit which can apply stress to the toes and overall forefoot. This can worsen multiple conditions such as bunions, hammertoes, Morton’s neuromas, and even ingrown toenails.</p>
<p>It’s also important to select the correct shoes for the activity you are doing. Athleisure shoes might be ideal for casual and relaxed hangouts but are likely to feel uncomfortable and cause unnecessary pain if standing for long hours, hiking, or playing any sort of impactful sport. By switching between different types of shoes, including those with more support, softer cushion, and a better fit, you can give your feet a break and reduce the risk of developing potential issues.</p>
<p>If you love athleisure style shoes like Vans but are concerned about their lack of support, there are solutions available. To better understand your current shoe size, it is essential to have both of your feet recently measured for the correct length and width.</p>
<p>Once you’ve gotten the correct measurements of your feet, the next step is to choose footwear that has a strong and supportive sole. With all the standing and walking you may do throughout a workday, it is important that your shoes maintain proper foot posture to keep you feeling comfortable and energized. Explore brands that provide sizes that closely match your foot measurements, including half sizes, narrow or wide widths, and maybe even extra depth to guarantee a comfortable fit throughout the day. You should also look for shoes that offer firm support around the heel and middle of the foot while resisting excessive side-to-side twisting. This doesn’t mean the shoes need to be completely stiff and rigid; in many cases, you can find shoes that strike a balance between strength and cushioning.</p>

These shoes are typically flatter and more snug on the sides, so be sure to choose a pair that fits comfortably with your thickest socks to ensure they aren’t too tight. 		<p>Cracked heels can quickly become a serious problem if they’re ignored.  Let’s dive in to learn why our skin must be protected and nurtured to keep it intact and doing its job!</p>
<h3><strong>The Skin</strong></h3>
<p>The skin is the body’s largest organ. Its primary job is to act as a waterproof barrier to protect everything underneath. There are 2 main skin layers, an inner layer with nerves and blood vessels and a protective outer layer. The skin on the bottom of our feet is extra durable to endure friction and pressure from the ground when we walk.</p>
<p>When the moisture balance of the skin is maintained, the skin remains strong and resilient. On the other hand, skin that is too dry or too moist becomes weak, and this can lead to cracks, breaks and even infection. Microorganisms like to live where it’s warm, dark and moist, our shoes and socks are the perfect environment for bacteria and viruses to flourish. They are also found on the surfaces we walk on every day.</p>
<p>In the summertime, many of us choose to wear sandals to keep cool when the temperatures soar. A supportive and well-structured sandal will protect the heel from the significant pressure it endures when it strikes the ground during gait. It’s important to remember that dry skin plus unsupportive footwear or walking barefoot are a recipe for disaster for our heels!</p>
<h3><strong>What can happen if your heels become cracked?</strong></h3>
<p>Although not common, if bacteria enters through cracks or <em>fissures</em> in the heel, it can travel into the blood and spread to surrounding tissues. <em>Cellulitis</em>, an infection and inflammation of the skin, may result. If left untreated this infection may lead to amputation or sepsis. Sepsis is a life-threatening condition, and it must be managed immediately in a hospital setting.</p>
<h3><strong>Let’s get and keep your heels healthy!</strong></h3>
<p>BioPed’s medical footcare clinicians recommend using a foot file on a regular basis to keep the skin around the heels soft and to also reduce thickness. The Footlogix FR Exfoliating Foot File is an effective tool to use at home to prevent cracks from forming in the first place. After filing your heels, it is beneficial to use a moisturiser that absorbs quickly, such as Footlogix Very Dry Skin or Footlogix Cracked Heel. Applied to moist skin, twice a day in colder months and once daily in warmer months is an excellent routine to follow. If you have callus around your heel, it is best to visit a BioPed Footcare Nurse or Registered Chiropodist for care. They will remove thick callus or dry flaky skin, allowing for better penetration of moisturisers to prevent future cracks from occurring. Pedicures at reputable salons (salons who disinfect and sterilize their tools properly) feel lovely, but they should not be a replacement for medical care or advice. People with diabetes often have issues with healing or a compromised immune system and for this reason, it’s best to avoid pedicures all together.</p>

<p>Properly fitting shoes can make a world of difference. Shoes are with us every step of the day. They are the interface that our feet use to interact with the ground, and they need to be given more care and consideration if we want them to work for us, instead of against us.</p>
<h3>Common complaints that keep us from doing more in our day:</h3>
<p><strong>Pain in the Toes</strong> – From slightly scrunched to blistered from friction, our toes are what push us from one step to the next, and these seemingly small discomforts can quickly stop us from doing more in our day. If you are kicking off your shoes before the end of the day and spreading out your toes, taping their blistered tips or seeing red pressure splotches all over them, your shoes are probably too tight. The properly fitted shoe has the depth, width and length that allows your toes to move without being pinched, squeezed or rubbed the wrong way.</p>
<p><strong>Pain at the Heel</strong> – Whether the pain is under the heel, through the heel, or right on the back, there are many shoe features that can help to alleviate these daily discomforts. When the shoe is too big the heel can slip around and even out of the shoe causing blistering and soreness along the tendons that rest there. Having a cushioned collar and deep heel cup to hold the heel in place can minimize these effects. Supportive, cushioned heels that are resilient as well as comfortable can increase shock absorption and decrease throbbing, achy heels.</p>
<p><strong>Pain at the Ball of the Feet</strong> – Corns, calluses, redness, swelling, soreness… any of these along the ball of the foot indicate that there is too much pressure happening in that bony area of the foot. This is the on-the-go movement, that push from the whole foot to the forefoot, and when it’s painful it drastically changes our yes to adventure, to a no from the couch. Properly fitting shoes that have a stiff forefoot rocker can help to propel you through your day with ease by offloading the front of the foot and lessening pain with each step.</p>
<h3>How do orthotics fit in?</h3>
<p>Quite literally, they fit into those new properly fitting shoes! When the shoe supports the foot, the shoe will also support the orthotic against your foot. 		<p>People become patients of BioPed Footcare for many different issues, but all for the same reason. They want to feel more comfortable and be able to do more in life. This month I sat down with a new client of mine who booked an initial assessment with me to talk about his foot pain. He mentioned that the pain on the sides and bottoms of his feet began when he changed his footwear recently.  He purchased a new pair of shoes for work which he felt were the right fit, but his foot pain gradually worsened after wearing them daily.  It became clear that his wider-set foot was being stuffed into ill-fitting shoes with a narrow toe box. Continuing to wear shoes that do not fit well or are not appropriate for your foot type could lead to biomechanical and structural issues.</p>

		<p>We had a discussion about his lifestyle and activity level.  His job is physically-demanding and he spends a significant amount of time on his feet during each shift.  I noticed corns on his 4th and 5th toes on both feet and he complained of a sharp shooting type pain in his left forefoot.  I suspected a neuroma which is a ball of nerve tissue that results from chronic irritation of the nerve between the metatarsals or long bones.  I performed a test to determine if he did in fact have a neuroma in his foot by squeezing the forefoot bones together from each side of the foot.  The test was negative.  The pain he felt in between these two metatarsal bones was likely due to a neuritis or inflammation of the nerve.</p>
<p>During his appointment, I treated the corns on his toes by removing the hard and thickened skin layer by layer.  This provided instant relief.   To complete his footcare treatment, I applied an emollient to soften the skin and restore moisture.   The second part of his appointment was a biomechanical assessment, which included evaluation of the <span class="proofing-commentary__text _ngcontent-jnh-24">range-of-motion of the joints in his feet, which were all normal. </span>My Pedorthist colleague and I conducted a gait analysis.  I like to watch my patients walk with one of the Pedorthists I work with because I value their perspective and skill set.  We consult with each other and problem solve how to best serve our patients.</p>
<p>Watching him walk, we noticed his arches were fallen and his feet appeared even wider. We decided to fit him for new shoes, by measuring the length and width of his feet.  He tried on a few different options and decided on a pair that provided comfort and stability.  The toebox was wide and allowed his toes and forefoot to splay and move without restriction.  These shoes gave him immediate relief from the pressure and pain he was experiencing.</p>
<p>I am thankful I met this particular patient and had the opportunity to speak with him about his foot pain.  Foot pain can be debilitating and can have a negative impact on work and life.  Two weeks after our initial appointment together, we saw each other again.  I was thrilled when he told me his feet were significantly better.  He was comfortable and the pain on the sides of his feet was dissipating every day. He was wearing his new shoes for work and was nowable to do more in his free time.  As a Registered Chiropodist, this is music to my ears!</p>
